Your life has suddenly changed because of stroke. You may be overwhelmed by information from your Stroke Book, and medical language can be confusing. It’s okay to be worried & have many questions, but it is important to learn all you can because 80% of strokes can be prevented from happening again!

StrokeApp is your mobile health coach or “personal nurse” that stays by your side, from the ER to hospital discharge. As featured in the Doctor’s Channel, Physician’s News Network, MedGadget and MobiHealth News, StrokeApp will provide lessons about stroke and explain how different areas of the hospital help in recovery. The inpatient education is customized to the particular type, location and severity of stroke. StrokeApp teaches when youre ready to learn with need-to-know information that is easy to understand. The educational content is individualized to your preferred learning style and remains relevant to where you are in the hospital and how far along you’ve come towards recovery.

The material in StrokeApp was prepared by Dr. Steven Cramer, a stroke specialist and internationally recognized leader in stroke rehabilitation. Currently, he is Professor and Vice-Chair of Neurology Research at University of California, Irvine School of Medicine, the co-editor of the book “Brain Repair after Stroke” and an Assistant Editor of Stroke (leading journal in cerebrovascular disease). StrokeApp is a recipient of the Top Promising Project Award of 2014 in the Big C Competition by LIVESTRONG, as well as the prestigious Box Collaboration Prize. StrokeApp was featured at the Health 2.0 conference in 2015 as an example of “Platforms for Patient-Provider Communications” that enhance the care continuum. Most recently, StrokeApp was a finalist in the Cambia Grove Anchor Partner Pilot program at EvergreenHealth.

personalRN values the patient-provider relationship and recognizes the importance of providing and maintaining a channel of accessibility and communication for patients and caregivers to their healthcare team. The meDiary, a feature of StrokeApp serves as that access portal as an inpatient and later after discharge. The meDiary also serves as storage, allowing you to collect and securely save all the information that is important to you during hospitalization. The meDiary can contain your personal health record and becomes your unique catalog, to be shared only with those you permit.
